Nerandomilast is a phosphodiesterase 4 (PDE4) inhibitor that preferentially targets the PDE4B subtype. It is an R-stereoisomer with one chiral center, and it exerts anti-fibrotic and immunomodulatory effects by elevating intracellular cAMP levels.
